New Patients

We want to welcome you to Allentown Pediatrics & Adolescent Medicine. We work hard to deliver excellent, compassionate medical care to our patients in a timely manner. To do this we ask you, as parents, to partner with us in this effort.

We are committed to helping you maintain and manage your child's healthcare needs. When you schedule an appointment with one of our MDs/PNP, that time is reserved exclusively for you to discuss and review your child's medical concerns. We do understand that on occasion unforeseen circumstances do arise and the need to cancel your appointment may be necessary. If you know that you are unable to keep your appointment, we ask you to show consideration by canceling your appointment at least 24 hours in advance. Providing our office with adequate notice will allow us to offer that appointment to another patient who needs to be seen by the physician. It is our practice's policy that if a patient no shows three appointments in a 6-month period of time the patient may be dismissed from the practice.

We have a PATIENT PORTAL. The patient portal allows secure electronic communication between the patient/parent/guardian and Allentown Pediatrics. You will be able to request and cancel appointments, request medication refills, view your child's immunization record and other aspects of the medical record, and request certain forms. Your provider will be able to share lab and test results as deemed appropriate.
  • You must be in the office to receive your activation letter.
  • You must have photo identification and a valid email address.
  • Due to confidentiality concerns, children 13-17 years will not be activated at this time.
  • Patients 18 years and older may activate the patient portal with the proper identification.

  • Telephone calls–Calls regarding appointments, medication refills, referrals, forms, lab or test results should be made during office hours only. Calls to discuss a sick child, behavioral or other issues should be made during office hours. The nurse or doctor will return the call in a timely manner.
  • After hours–If your child is having a medical problem that cannot wait until the morning, please call the office and you will be forwarded to the answering service. The on-call doctor or nurse will return your call. We ask that you call for advice before taking your child to an urgent care facility or emergency room. In many cases the doctor will be able to provide home care advice that will make a trip to the emergency room unnecessary. IF YOUR CHILD IS HAVING A LIFE THREATENING EMERGENCY, CALL 911.
  • Insurance–Please make sure to bring in your child's insurance card at the time of visit. If your insurance changes, we require that you notify the office and bring in a new insurance card at your next appointment.
  • Photo ID–Please bring photo ID with you to your appointment. We will scan it into your child's chart for future reference.
  • Co-pays–If you have a co-pay, payment is required at the time of service. We accept VISA, MasterCard and Discover for your convenience along with cash or a check.
  • Permission slip–Ideally children should be accompanied by their parent or guardian at all appointments. If for any reason you are unable to bring your child, please complete the permission slip (download form here) authorizing the person of your choice to bring your child to see us. Photo ID will be required by the person authorized to bring your child/children to any appointments.
